New Jersey Wraparound Services

At Neuropath Behavioral Healthcare, we are proud to offer Wraparound Services as part of a New Jersey State and federally funded framework designed to support children and their families.

Our comprehensive approach ensures that youth with complex emotional, behavioral, and mental health needs receive the coordinated care they need to thrive. These services are provided in collaboration with the New Jersey Department of Children and Families (NJ DCF) and the Children’s System of Care (CSOC) to build a seamless support network for children and adolescents.

Wraparound Services

These services are designed to be comprehensive, individualized, and family-centered, ensuring that all aspects of a child’s life are supported in a way that promotes long-term well-being. The range of services includes:

Mental Health Counseling – Therapy and counseling to address behavioral and emotional health challenges.
Case Management – Intensive care coordination and individualized service planning for youth with complex needs.
Educational Support – Advocacy and connection to resources to help children overcome academic challenges.
Family Advocacy– Peer support, education, and advocacy to empower families throughout the care process.
Community Resources – Access to local support services, ensuring a complete network of care.
Behavioral Health Services – Therapy, counseling, and psychiatric support for mental health needs.
Intellectual/Developmental Disability Services –Tailored support for youth with developmental or intellectual disabilities.
Substance Use Treatment Services – Comprehensive treatment options, including outpatient and inpatient care.
In-Home and Community-Based Supports – Interventions designed to maintain stability and promote healthy development in home and community settings.
Residential and Out-of-Home Treatment Services – Intensive care when more specialized treatment is required outside the home.
Transitional and Aging-Out Support – Resources to help youth transition to adulthood, including assistance with independent living through Wraparound Funds.
Crisis Intervention and Stabilization– 24/7 support teams ready to assist in managing acute behavioral health crises.
Mentoring and Life Skills Programs– Guidance for personal growth, career planning, and independent living.
Respite Care Services – Temporary relief for caregivers, allowing them to recharge while their child receives needed care.

The Wraparound Model of Care

Each Care Management Organization (CMO) in the CSOC framework uses the Wraparound model of care, which is based on ten guiding principles that ensure a well-rounded, effective approach:

Family Voice and Choice
The perspectives of both youth and family are central to the planning process. The team respects the family’s values and preferences in creating a care plan.
Team-Based
The Wraparound team is composed of individuals chosen by the family, including both formal service providers and informal supports from the community.
Natural Supports
The team actively includes support from the family’s network, such as extended family, friends, and community members, ensuring a well-rounded support system.
Collaboration
Team members cooperate to develop and implement a single, coordinated care plan that blends different perspectives and resources toward achieving shared goals.
Community-Based
Services are provided in the most inclusive, responsive, and least restrictive settings, promoting integration into home and community life.
Culturally Competent
The Wraparound process acknowledges and respects the cultural values, beliefs, and identity of the child, youth, and family.
Individualized
Each plan is uniquely tailored to the child and family’s needs, ensuring that services are customized to their specific circumstances.
Strengths-Based
The team focuses on building and enhancing the strengths, skills, and assets of the child and family, fostering their capacity for growth and success.
Unconditional
The team remains committed to the child and family, even through challenges, and continues to work toward achieving the agreed-upon goals.
Outcome-Based
The Wraparound plan includes measurable goals, and progress is tracked. If necessary, the plan is adjusted to ensure the best possible outcomes.

Key Services Available through the Children's System of Care

Through the Children’s System of Care (CSOC), we also provide a range of specialized services, including:

Children’s Mobile Response and Stabilization Services (MRSS) – Available 24/7 to respond to emotional or behavioral crises and offer immediate stabilization.
Family Support Organizations (FSOs) – Provide peer support, advocacy, and resources for families navigating the system.
Children’s Interagency Coordinating Councils (CIACCs) – County-based planning bodies ensuring coordinated service delivery.

Additionally, we offer services in areas like substance use treatment, developmental disability services, and residential care, as well as support for youth transitioning into adulthood.
